What is a premill custom abutment?
A premill custom abutment is a patient-specific implant abutment that is CAD/CAM-milled from a titanium blank whose implant connection is already manufactured industrially. Because the connection comes prefabricated, the precision of the implant interface is preserved, while the upper part is designed entirely for the individual case. Unlike stock abutments, you define the emergence profile, the margin position and any angle correction for each patient.
Indications
Premill custom abutments are the choice for single and multiple implant restorations where a stock abutment cannot deliver an ideal result.
- Cases in which a stock abutment cannot create the correct emergence profile
- Angled implants that require axis correction
- Aesthetic zone restorations where the margin must follow the gingival level
- An ideal supporting shape for cemented zirconia or e.max crowns
- Deeply placed implants where excess cement must be easy to remove
Material and production
We design the abutment in 3Shape or Exocad, evaluating the scan or model data of the implant position together with the form of the final restoration. The crown is planned first and the abutment is reduced from that shape, so the restoration has an even material thickness in every direction. The emergence profile is shaped to support the gingival contour; the margin is placed slightly subgingival in the aesthetic zone and at a level that makes cement removal easy in the posterior region. The design is milled from a titanium premill blank on our Arum 5-axis CAD/CAM unit and the surfaces are finished. Because the zirconia, e.max or metal-ceramic crown is produced from the same digital data, both parts fit together precisely.
Why Marjinal CAD?
We plan abutment and crown as one restoration rather than two separate parts. For a dental laboratory working on implant cases, this consistency is decisive for long-term soft-tissue health and aesthetics.
- Titanium premill blanks with prefabricated implant connection
- Arum 5-axis CAD/CAM milling
- Abutment designed back from the crown form
- Patient-specific emergence profile and angle correction
- Abutment and crown produced from the same digital data
Sending a case
An intraoral scan with a scan body (iTero, 3Shape TRIOS, Medit; STL or PLY) or an implant impression is all we need. To transfer the gingival profile accurately, we recommend adding a soft-tissue scan taken with the healing abutment removed. For blank selection, the implant brand, system, platform diameter and connection type are mandatory; we confirm with you before design whether a suitable blank is available for your system. Frequently asked questions
How does a premill abutment differ from a stock abutment?
A stock abutment has a standard, ready-made shape. A premill abutment keeps the prefabricated implant connection, while its upper part is milled with a patient-specific emergence profile and angulation.
Can premill abutments be made for every implant system?
Premill blanks are manufactured with a system-specific connection. We confirm with you before design whether a suitable blank exists for your system.
Which crowns can be placed on a premill abutment?
Zirconia, e.max or metal-ceramic crowns. Since crown and abutment come from the same digital data, they fit together precisely.
What information do you need for a case?
An intraoral scan with a scan body or an implant impression, plus implant brand, system, platform diameter and connection type.
